Ok, based on the original criteria of this post, here's one that probably EVERYONE overlooked; the Not Fade > Going Down the Road from the Skull n' Roses (Skullfuck, if you will)album. It's an unbelievable transition that is completely seamless. Listen to the chord progression Bob does to get you there. Why they didn't do it this way (even when they paired these two) in the future is beyond me. It is incredibly well done (even for an album :) )
